Visual Outcomes After Implantation of Diffractive Multifocal Intraocular Lens
Abstract
Purpose : To assess the visual outcomes after diffractive multifocal intraocular lens (IOL) implantation. Methods : Twenty-five subjects (48 eyes) who underwent implantation with the diffractive multifocal IOL were analyzed at 1 week, and 1, 3 and 6 months after cataract surgery. Uncorrected visual acuity(UCVA) at distance, near, and intermediate was measured and a satisfaction questionnaire regarding visual outcomes was conducted after surgery. Results : The mean subject age was 66.23±6.02 years. Pre-op spherical refraction averaged +1.33±1.78 D, with astigmatism of -1.01±0.58 D. Preoperative uncorrected distance visual acuity (UCDVA) was 0.75±0.27 logMAR. At 1 week postoperatively, UCDVA improved to 0.10± 0.10 logMAR, uncorrected near visual acuity (UCNVA) to 0.14±0.10 logMAR, and uncorrected intermediate visual acuity (UCIVA) to 0.22±0.12 logMAR. These improvements were sustained at 6 months, with UCDVA 0.08±0.08 logMAR, UCNVA 0.12±0.09 logMAR, and UCIVA 0.20± 0.11 logMAR. At 1 week, spherical refraction was -0.02±0.50 D and astigmatism was –0.71± 0.47 D. By 6 months, sphere was +0.08±0.49 D and astigmatism -0.77±0.48 D. These represented statistically significant reductions compared with preoperative values (p<0.001 for sphere, p=0.004 for astigmatism). Overall satisfaction after cataract surgery was 83.9%. Satisfaction with UCDVA, UCIVA, and UCNVA was 76.1%, 65.8%, and 73.3%, respectively. Conclusion : This study found that the UCDVA showed the best outcomes, while UCIVA was relatively lower compared to both distance and near VA, though it remained at an acceptable level. These clinical outcomes suggest that mutifocal diffractive IOL represent a reliable solution for appropriately selected cataract patients.
